Application Checklist.

All students applying for clerkships must use the Exchange Clerkship Application Form. You will also need to send certain supporting documents to the Medical School at Francisco Marroquín University. We will not process your application until all of the required documents are received. We must have your complete application at least 60 days in advance of the clerkship start date but no earlier than 6 months prior to the start date.

The following checklist will help ensure that you do not forget anything during the applications process. You may want to print out this page as a reminder.

  1. Download online Exchange Clerkship Application Form. Download here.
  2. Check application deadlines: Be sure to send no prior of 6 months and no later than 60 days before the date your clerkship starts.
  3. Fill out the application form: The form must be complete to be process. It requests biographic information, emergency contact information, medical school information, health information and your clerkship selections. After filling out the application form, please print it out before submitting it to us and scan it to be send via email.
  4. Gather the following supporting documents to send: These documents should be sent with your application.
  • A Dean’s Statement from your School of Medicine Dean. You may print out the Dean´s Statement and have it filled out and signed by your Dean. Which is the section 3 of your application form (fourth page).
  • Proof of dates of required immunizations: Rubella and mumps, Measles, Varicella, Hepatitis B, and Tuberculosis.
  • A proof of student covered with personal health insurance.
